Nav-CARE

Nav-CARE is a free, research-based program designed to help individuals and families navigate the effects of declining health and life-limiting illnesses. Often adults living at home with a serious illness or declining health, particularly those who do not yet qualify for home-based nursing care, live with unmet needs— resulting in a poor quality of life. Nav-CARE addresses these issues by connecting individuals to the services and resources needed to navigate their changing health and enhance their quality of life.

Like a friendly neighbour or helpful family member, specially trained Nav-CARE volunteers visit individuals in their home on a regular basis to:

• Help to relieve feelings of loneliness, isolation, and anxiety

• Assist to re-engage in hobbies and other interests

• Help locate local community services and resources needed to manage declining health.

• Book appointments

• Talk you through important decisions and assist in planning for the future.

Nav-CARE (Navigation: Connecting, Advocating, Resourcing, Engaging), is a program offered at no cost that supports people living with declining health to live well at home for as long as possible. Specially trained volunteer navigators help persons and their families access resources and services in their community, while providing companionship and emotional support.
